MoKP CKD Educational Award for FY16

MoKP is pleased to announce the continuation of the CKD Educational Silver award directed to improve the health and well-being of MO CKD patients. Donated funds will be funding these educational and preventive efforts. The educational activity is to be developed, and implemented by the requesting facility or health agency. Examples are:

  • Dietary education with cooking instructions, or trips to grocery stores/farmer's markets.
  • Hypertension education and sodium management lessons, with BP cuff.
  • Medication adherence education with digital reminder pill boxes.
  • Spanish educational materials to promote health literacy.
This award is not intended for an individual need, but a group or clinic project. It can only be requested once a year and is a reimbursement only process. Gift Card purchases will not be allowed.

The Silver Award will be up to $500, and will be focused on short-term education efforts (1-4 months). Applications can be submitted and approved through Jan. 15, 2016. Use form below.

The Gold Award will not be available in FY16.

Future funding amount and policies will be reviewed and approved yearly by the Director and MoKP Advisory Council.

Process:

1. Applications will be submitted to the MoKP Director: Leanne Peace at peacelj@health.missouri.edu
2. Director will review and approve the Silver application within 10 business days.
3. Awardee will purchase items first. Gather and submit all bills together to MoKP.
4. Reimbursement will be processed in one lump sum after all receipts for the project are received.

6. An end-of-project report with outcomes summary is required.
7. All final bills and reports are to be submitted by May 31, 2016.
8. MoKP is to be recognized as funding sponsor, and listed on any educational materials developed or publications.

Cost Containment Research

CALL FIRST TO SEE IF FUNDING IS AVAILABLE

The Missouri Kidney Program (MoKP) is interested in funding pilot proposals that will help contain the costs of care related to end-stage renal disease (ESRD). Proposals must be designed to have direct cost-containment application. The reasonable expectation is that at the conclusion of the study information will generate clear cost-reducing potential for MoKP programs and/or participants.

Cost Containment Objective

1. Increase transplant availability, organ donation consent rates, and graft survival.
